Yes we've had the same problem. On ours we blamed it on the idler bracket getting bent when we reversed the head putting pressure on it. I added some angle iron to the bracket and put a bolt through the idler that we can get good and tight. The carriage bolt had spun out the slot and wouldn't hold. Be sure the chain and sprockets are in good shape. That chain pulls off of a pretty small sprocket so it needs to be in pretty good shape to stay together.
